SUBCONTRACTOR PREQUALIFICATION

All Subcontractors Must Review This Document Before Submitting a Prequalification Form.

*The following items are REQUIRED to complete this form and finalize the prequalification process. Incomplete forms will NOT be accepted.

1. COMPANY GENERAL INFORMATION
   Address, main phone, primary contacts, accounting contact(s), structure, trades, license #s, etc.
2. W-9 TAX FORM (current)
3. SAFETY & HEALTH INFORMATION
   EH&S contact name & #, EMR, OSHA 300A data for the previous 3 years, and OSHA violations.
4. S&H MANUAL
   Written safety programs or company safety policies and IIPP.
5. CERTIFICATE OF LIABILITY INSURANCE
   (Evidence of Insurance or Project-Specific) with endorsements.
6. OTHER RELEVANT DOCUMENTS YOU MAY WISH TO SUBMIT
   Financial Statements and Bonding Capacity Certification Letter, NDAs, OSHA 300 logs, JHAs, training records, explanation letter for any company citations/violations.
